As salamu alaikum,

I have read about Taweez, only those from Quran and Sunnah are permissible, other ones that involve giving instructions to jinn are not allowed, is this correct, are there conditions that have to be met for a Taweez to be permissible?

Usage of Jinn is permissible for permissible things. Taweez must have no blasphemous elements. One's beliefs must be totally valid - that it is not the medication or the Taweez that does and cures, it is Allaah.
